Many Ontarians don’t currently have a main health care provider, and it can be difficult to find one that is taking new patients.
Moving to a new town may mean your current doctor is too far away for visits to be practical.
If you don’t have a family doctor, or your current one is too far away, you can sign up with Health Care Connect. This service is operated by the Ministry of Health and helps you find a new doctor in your area.
You can register online or by telephone.
To register at Health Care Connect, you must have a valid Ontario health card and an up-to-date mailing address linked to your health card.
You will need to provide your name and address, your health card number, and basic health information.
You can sign up for yourself, your family, or someone else with their permission.
Once you register, a nurse will review your request. They will try to find a doctor or nurse practitioner near you.
You will be contacted if a match is found and may be connected to local health services while you wait.
Finding a suitable match may take some time. People with urgent health needs are prioritized, so be patient.
If you decline a match, you will need to register again to keep looking.
